Home Health Agencies participating in Pre-Claim Review are having claims incorrectly returning with reason code 39621, indicating to remove the UTN. Agencies should not remove the UTN! This issue has been reported and is in research. Palmetto GBA is actively working on a solution to correct the issue. No provider action is required at this time. Home health agencies should not remove the unique tracking number (UTN). Palmetto GBA will provide updates when available.
